How to Get a Group Quote from Dillon Health:
- Download the Census Form available below or if you have your own spreadsheet we may use it instead as long as it includes date of births and zip code for every employee on the plan. Also if a spouse and/or child(ren) are to be covered, please include their date of births as well for each dependent.
- In addition, please provide your current plan renewal information so we may quote the correct benefits and rates.
- Return completed Census Form or your own spreadsheet with requested information to info@dillonhealth.com or via secure fax at 775-201-1794.
